Sandy Ridge Center for Rehabilitation & Healing is a 60-bed Skilled Nursing Facility that specializes in short-stay rehabilitation and long-stay care. We are dedicated to providing “Quality Focused Care, One Resident at a Time” as our mission states. We accomplish this through our focus on the individual needs of our residents and patients and a successful partnership with our residents, families, physicians and fellow providers.
Our experienced and dedicated interdisciplinary team has allowed us to experience favorable outcomes in our successful discharge to community rates, return to hospitalization rates, and increase in functional ability statistics.
Sandy Ridge Center for Rehabilitation & Healing has been a part of the Milton community for three decades and continues to strive to provide the highest levels of customer service and resident-centered care to its members. We are conveniently located within the heart of Milton and within miles of Santa Rosa Medical Center and many other key healthcare providers.
Our Medical Director and attending physicians all have offices located in Santa Rosa county and are equally invested in promoting health and wellness for the members of our community.
We currently offer private and semi-private rooms. We accept Medicare, Medicaid, various Managed Care plans and Private Pay residents and patients.
